29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems.      a) At 6232 Fly Road, East Syracuse, NY., on or about 2/27/2018: The employer did not ensure workers were protected from fall hazards while performing roofing work at approximately 11 feet above the ground level.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access.   a) At 6232 Fly Road, East Syracuse, NY., on or about 2/27/2018:  The employer permitted ladder use without ensuring safe access onto the roof, as the side rails of the ladder were positioned approximately 5 inches below the upper landing surface, exposing workers to fall hazards of 12 feet to the ground.
