29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2m) or more above a lower level was protected from falling:  Outdoor yard, next to concrete wash pad:  On or about February 28, 2018, and at times prior thereto, employees were exposed to a fall hazard greater than 4 feet to the dirt ground below while accessing the top of a frac tank while starting a 2 cycle gasoline engine and gauging the level of liquid in the frac tank.
