29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    (a) The employer did not ensure that a ladder that could not extend at least 3 feet above the upper landing surface was secured and that employees were provided a grasping device to assist during mounting and dismounting of the ladder.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) The employer did not ensure that employees were using fall protection when conducting roofing work on a residential building.   Paul Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 966227, citation number 01, item number 02 and was affirmed as a final order on June 3, 2014, with respect to a workplace location at N43 W22772 Victoria St., Pewaukee, WI 53072.
