29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    a.  On or about February 27, 2018, at the above addressed jobsite, an employee was exposed to falls while performing carpentry operations from a welded framed scaffold, approximately ten (10) feet above the working surface, that did not have the working level fully planked.

On or about February 27, 2018, at the above addressed jobsite, an employee performing carpentry operations from a welded frame scaffold, approximately ten (10) feet above the ground, was exposed to injuries from falls due to the lack of guardrails.

On or about February 27, 2018, at the above addressed jobsite, an employees performing residential framing operations on a roof fourteen (14) feet five (5) inches above the ground, was not protected from falls, thereby exposing the employee to fall hazards.    b.  On or about February 27, 2018, at the above addressed jobsite, employees were performing residential framing operations in a townhouse that did not have guardrails around the basement access, thereby exposing the employee to fall hazards.

On or about February 27, 2018, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record.
