29 CFR 1910.37(b)(2): Each exit must be clearly visible and marked by a sign reading "Exit."  On or about February 28, 2018, the exit located on the northeast corner was not labeled as an "Exit".  IN THE ALTERNATIVE  29 CFR 1910.37(b)(5): Each doorway or passage along an exit access that could be mistaken for an exit must be marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use (e.g., closet).  On or about February 28, 2018, the exit located on the northeast corner was not labeled as "Not an Exit".  Dollar Tree Stores, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.37(b)(5), which was contained in OSHA inspection number 1175394, citation number 1, item number 1 and was affirmed as a final order on November 1, 2016, with respect to a workplace located at 1100 Columbia Center, Columbia, IL 62236.  Dollar Tree Stores, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.37(b)(5), which was contained in OSHA inspection number 1008888, citation number 1 , item number 1  and was affirmed as a final order on January 11, 2016, with respect to a workplace located at 1428 West Grey in Houston, Texas.  Dollar Tree Stores, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.37(b)(2), which was contained in OSHA inspection number 992095, citation number 1, item number 2 and was affirmed as a final order on October 1, 2014, with respect to a workplace located at 2721 Bronxwood Ave., Bronx, NY 10469.

29 CFR 1904.40(a): Basic requirement. When an authorized government representative asks for the records you keep under Part 1904, you must provide copies of the records within four (4) business hours.  On or about February 28, 2018 the OSHA 300 Log of Work-Related Injuries and Illnesses and OSHA 300A Summary of Work-Related Injuries and Illnesses for calendar year 2015 - present were requested for review.  The logs were not provided until March 8, 2018.
