An employee was exposed to point of operation on the #5 Automated Leadmaker, Komax 40-S, serial #5779.    b.) An employee was exposed to the point of operation on the #8 Automated Leadmaker, Komax 40-S, serial #5056.    c.) An employee was exposed to the point of operation on the #38 Crimper Press, Molex ATP-201, serial #5003.    d.) An employee was exposed to the point of operation on the #39 Crimper Press, Molex ATP-201, serial #20112.

29 CFR 1910.213(c)(1): Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:  On or about January 26, 2018, the employer failed to ensure that the table saw in the mezzanine of the engineering department was adequately guarded. The saw-blade did not have an automatically adjusting hood guard.

29 CFR 1910.1200(e)(1)(ii): The written hazard communication program did not include the methods the employer would use to inform employees of the hazards of non-routine tasks, and the hazards associated with chemicals contained in unlabeled pipes in their work areas:     On or about March 1, 2018, the employer failed to develop the section of the written hazard communication program that includes the information related to non-routine tasks. Employees are exposed to chemicals at the worksite, including but not limited to: solvents, degreasers, compressed gases, fuel gas, paint, lubricants, and aerosol sprays.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours:    On or about January 26, 2018, an employee was injured while operating Table Saw #1. The employee was admitted to a hospital due to his injuries. The employer failed to notify the local area OSHA office of an employee in-patient hospitalization as a result of a work-related incident within twenty-four (24) hours.
