29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  a) On or about 02/27/2018, at the salvage yard, employees operating a Clark forklift, model C25CL, to handle auto parts such as, but not limited to 4-cylinder motor engines, had not been trained and evaluated by the employer.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:  a) On or about 01/24/2018, at the salvage yard, an employee pinched his right hand with a forklift mast chain while picking up an auto engine. The employee was admitted to the hospital on 01/24/2018, and on 02/02/2018, the employer found out that the employee's middle and ring fingers were surgically amputated. The hospitalization and amputation were not reported to OSHA.
