29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a. On or about 2/20/2018 at 1560 South Main Street Greensboro, GA, workers performing cutting operations with the Milwaukee portable band saw were exposed to electrical shock and burn hazards where the grounding pin was missing. b. On or about 2/20/2018 at 1560 South Main Street Greensboro, GA, workers of all trades were exposed to electrical shock and burn hazards where the yellow extension cord that was plugged in and available for use was missing the grounding pin.
