29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.    The employer did not ensure that the walking-working surfaces throughout the work area were free of pot holes. Employees walking in the area and using mechanical equipment were exposed to tripping  and tip over hazards.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s)    In the sandblasting area, the employer did not ensure that the tires on the Toyota forklift were repaired or replaced. Employees were operating the Toyota forklift with damaged tires exposing employees to a struck by hazard.

29 CFR 1910.1200(f)(5): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the identity of the hazardous chemical(s) contained therein and the appropriate hazard warnings:      The tanks of sodium hydroxide were not labeled with the identity and hazard warning for employees performing tasks such as, but not limited to cleaning aluminum dies.  Employees were potentially exposed to skin burns and respiratory inhalation hazards.

29 CFR 1910.157(c)(1): The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury.  In the sandblast area, the Ansul portable fire extinguisher location was not signed or labeled exposing employees to the hazard of smoke inhalation and fire.
