29 CFR 1910.137(c)(2)(ii): Insulating equipment was not inspected for damage before each day's use and immediately following any incident that can reasonably be suspected of causing damage.  Insulating gloves shall be given an air test, along with the inspection:    Within the Production Department located at 4328 Bridgeton Industrial Park Drive, Bridgeton, Missouri:      Insulating gloves were not inspected for damage before each day's use for employees performing electrical tests with voltages ranging between 120 VAC and 2,500 VAC, during the following, but not limited to, forklift battery charger production processes:  a.  The in-process inspection procedures for transformers,   b.  The final assembly electrical tests.

Within the Production Department located at 4328 Bridgeton Industrial Park Drive, Bridgeton, Missouri:    Protective gloves were not worn over insulating gloves to prevent damage for employees performing electrical tests with voltages ranging between 120 VAC and 2,500 VAC, during the following, but not limited to, forklift battery production process: a.  The in-process inspection procedures for transformers,  b.  The final assembly electrical tests.    In the alternative,  1910.335(a)(1)(iii) When the insulating capability of protective equipment may be subject to damage during use, the insulating material was not protected.

Test voltages and the maximum intervals between tests shall be in accordance with Table I-4 and Table I-5:  Within the Production Department located at 4328 Bridgeton Industrial Park Drive, Bridgeton, Missouri:    The employer did not ensure that electrically insulated gloves that were provided to workers had been tested prior to first issue and within the maximum intervals in accordance with Table I-5.

29 CFR 1910.333(c)(2): The employer did not ensure that qualified persons were capable of working safely on energized circuits and were familiar with the proper use of special precautionary techniques, personal protective equipment, insulating and shielding materials, and insulated tools.    Location:  Within the Production Department located at 4328 Bridgeton Industrial Park Drive, Bridgeton, Missouri:      Employees performing component electrical tests during the production of forklift battery chargers were not familiar with the proper use of precautionary techniques and personal protective equipment required while performing electrical tests with voltages ranging between 120 VAC and 2,500 VAC, during the following, but not limited to, forklift battery production process:  a.  The in-process inspection procedures for transformers,   b.  The final assembly electrical tests.
