29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a) At Schillinger Rd and Lamplighter Rd, Semmes, Alabama (Intersections/Roadway Area):  On or about February 27, 2018, and at times prior thereto, the employer failed to instruct or train their employees in recognizing, eliminating or controlling hazards associated with work performed in an active roadway thus exposing the employee(s) to struck-by hazards from vehicular traffic.

29 CFR 1926.201(a): Signaling by flaggers and the use of flaggers, including warning garments worn by flaggers did not conform to Part VI of the Manual on Uniform Traffic Control Devices, (1988 Edition, Revision 3 or the Millennium Edition),     a) At Schillinger Rd and Lamplighter Rd, Semmes, Alabama (Intersections/Roadway Area):  On or about February 27, 2018, and at times prior thereto, the employer failed to provide high-visibility apparel to employees working as flaggers in traffic work zone thus exposing them to struck-by hazards from vehicular traffic.
