29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:  a) On or about 2/27/18 at a house renovation project located at 17 Orchard Avenue, Angola, New York; employees were replacing the roof of a two story house: employees using a scissor lift to access the roof were climbing over the guardrails to get on and off the roof. Employees were exposed to falling approximately 16 feet to the driveway.  NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) On or about 2/27/18 at a house renovation project located at 17 Orchard Avenue, Angola, New York; employees were replacing the roof of a two story house. The roof had a 6 in 12 pitch and an eave height of approximately 16 - 18 feet. There was no fall protection in use.  NO ABATEMENT CERTIFICATION REQUIRED
