29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.    On or about March 5, 2018 in the wire wrap department employees were exposed to struck by hazards when the transfer cart operation system for moving pipe was not adequately guarded.

29 CFR 1904.33(a): The employer did not save the OSHA 300 Log, the privacy case list (if one existed), the annual summary, and the OSHA 301 Incident Report Forms for five (5) years following the end of the calendar year that these records covered.   On or about March 7, 2018, the employer did not retain an OSHA 300 Log for five (5) years following the end of the calendar years 2014, 2015 and 2016.
