29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.  a) Jobsite: The employer did not ensure that guardrail systems were installed along all open sides and ends of platforms. Employees were working at heights greater than 10 feet without a guardrail system installed along the open sides of the platform, thus exposing employees to fall hazards.  Condition noted/observed on/about 20 FEB 2018.

29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:  a) Jobsite: The employer did not ensure that each scaffold platform was at least 18 inches wide. Employees were working from an outrigger which only contained 1 board, approximately 9 inches wide; thus exposing employees to fall hazards.  Condition noted/observed on/about 20 FEB 2018.
