29 CFR 1910.134(c): The employer did not develop and implement a written respiratory protection program with required worksite-specific procedures and elements for required respirator use: (Construction Reference 1926.103)  On or about 02/28/2018, at 900 Bearcat Drive, Ruston, LA 71270, an employee was operating a saw to cut cement blocks and was required to wear a respirator. The employer had not developed and implemented a respiratory protection program.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  On or about 02/27/2018, at 900 Bearcat Drive, Ruston, LA, an employee was using a saw to cut cement blocks with a electrical cord that did not provide sufficient strain relief. The outer protective sheath had been pulled away exposing the secondary coverings.
