29 CFR 1926.416(a)(3): Before work began, the employer did not ascertain by inquiry, direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring any person, tool, or machine into physical or electrical contact with the electric power circuit. The employer did not ensure that the electrical lines from the service drop were located so that employees working to install asphalt shingles on the roof would not come in contact with electrical power circuit from a 120/208v triplex service drop.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. The employer did not ensure fall protection was implemented during commercial roofing operations when workers were exposed to the hazard of falling approximately up to ten (10) feet off the roof eve when they were installing asphalt roof shingles.
