29 CFR 1910.219(e)(1): 1910.219(e)(1)(i) Horizontal belts which had both runs 42 inches or less from the floor level were not fully enclosed by guards conforming to the requirements specified in 29 CFR1910.219 (m) and (o): On or about 3/8/18, the employer failed to ensure that belts, including but not limited to the puller belts on line #4, had guarding methods in place. The puller was completely open exposing the belts ingoing nip point, and the sides.

29 CFR 1910.303(b)(7): Electric equipment was not installed in a neat and workmanlike manner: On or about 3/8/2018, the employer failed to ensure that electrical connections, including but not limited to, the heater bands attached to the extrusion dies, on extruder lines #1 through #7, were installed in a workmanlike manner. The heater band conductors were connected with wire nuts, and the wires were secured to the machines with metal Irwin spring clamps.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): On or about 3/8/2018, the employer failed to ensure that electrical connections, including but not limited to, the 240V heater bands attached to the extrusion dies, on extruder lines #1 through #7, were guarded against accidental contact. The heater band conductors were connected with wire nuts, and the wires were secured to the machines with metal Irwin spring clamps.
