29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent.   (a) On or about March 7, 2018, the employer had not recorded the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2018:   - the February 13, 2018 arm fracture injury that occurred when the tire of a small tow truck rolled over an employee's arm when working on the brakes, causing the employee to be hospitalized for three days.  (b) On or about March 7, 2018, the employer had not recorded the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2016: - the November 2016 right index fingertip amputation injury that occurred when the employee hit the finger with a tool and was hospitalized for three days.
