29 CFR 1904.41(a)(2):  The workplace establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, with a classified industry listed in appendix A to subpart E of this part.  The establishment was required to electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee, but did not do so.  The information must be submitted once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form.  a) Door Services Corporation - On or about March 6, 2018, the establishment employed 38 employees with a NAICS of 238290 during calendar year 2016 and the employer failed to electronically submit information from their 2016 OSHA Form 300A or equivalent by December 15, 2017.  No abatement certification or documentation required.

29 CFR 1910.1200(f)(6):  Workplace labeling. Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer shall ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with either:  (i) The information specified under paragraphs (f)(1)(i) through (v) of this section for labels on shipped containers; or, (ii) Product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.  The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked as required.  a) Door Services Corporation - On or about March 6, 2018, employees were adding Shell Spirax automatic transmission fluid to automatic door openers from unlabeled containers.  The containers in use did not specify the identity of the chemical, nor the hazards associated with hydraulic oil, exposing employees to eye and skin hazards.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
