29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents: a) On March 9, 2018, employees were exposed to eye injury while operating a gas powered saw without eye protection during masonry work. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On March 9, 2018, employees engaged in masonry work were not protected from fall hazards of approximately 40 feet to the ground, while working along unprotected edges of the deck. b) On March 9, 2018, employees were not protected from the hazard of falling approximately 10 feet to the lower level through unguarded deck opening. c) On March 9, 2018, employees were exposed to the hazard of falling approximately 40 feet to the ground while working in the hoist area. No abatement certification or documentation is required for this item.
