On November 27, 2017, in the Tire and Lube Express Area, an employee was utilizing a Coats X-Series 70X-AH-3 Rim Clamp Tire Changer in order to mount a 14 inch tire on a 14 ½ inch wheel rim.  While in the process of seating the bead of the tire with air from an unregulated source at a pressure exceeding 40 pounds per square inch (psi), the tire exploded.
