29 CFR 1904.29(b)(1): 29 CFR 1904.29(b)(1): A log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation:  a) Description(s) of recordable injuries on the 2017 OSHA 300 Log (Column F) did not include information on the object or substance causing injury.  Among the recordable injuries with object / substance omitted from the description in Column F was 2/23 shoulder strain with 170 days away, 3/10 neck strain injury with 14 days away, 5/02 lower back strain with 19 days away, 5/04 wrist sprain with 109 days away, 5/12 knee contusion with 24 days away, 7/25 shoulder strain with 93 days away, and 9/29 lower back strain with 11 days away.    b) Description(s) of recordable injuries on the 2016 OSHA 300 Log (Column F) did not include information on the object or substance causing injury.  Among the recordable injuries with object / substance omitted from the description in Column F was 1/05 hernia injury with 43 days away, 3/15 toe(s) contusion with 3 days away, 5/06 ruptured spinal disc with at least 180 days away as well as lower back strain injuries on 5/17, 8/10, 11/04/ and 11/08 with 43, 19, 22 and 53 days away respectively.    c) Description(s) of recordable injuries on the 2015 OSHA 300 Log (Column F) did not include information on the object or substance causing injury.  Among the recordable injuries with object / substance omitted from the description in Column f was 1/06 head injury with 139 days away, 4/16 abdominal / groin strain with 42 days of work restriction or job transfer, 4/22 leg fracture with at least 180 days away, 6/09 crushed finger(s) injury with 20 days away, 7/03 crushed foot injury with at least 180 days away, lower back strain injuries on 8/18 and 8/25 with 115 and 128 days away respectively, and 10/05 hand fracture injury with 1 day away and 19 days of work restriction or job transfer.  Description(s) of recordable injuries on the 2014 OSHA 300 Log (Column F) did not include information on the object or substance causing injury.  Among the recordable injuries with object / substance omitted from the description in Column f was 2/21 herniated abdomen / groin with 70 days away and 30 days of work restriction or job transfer, 8/27 wrist contusion with 74 days away, and 11/25 finger(s) fracture injury with 6 days away.  Condition(s) were noted on or about 3/13/2017.
