29 CFR 1926.1053(b)(1): The side rails of portable ladders used for access did not extend at least 3 feet above the upper landing surface, or was secured at its top to a rigid support that will not deflect and along with a grasping devise to assist employees mounting and dismounting the ladder:  a) 556 East Broadway: On March 13, 2018 and at times prior, Perschon Associates, Inc., as the controlling employer, did not ensure that its sub-contractor Serrano Roofing's employee(s) were protected from fall hazards while using a ladder that did not extend 3 feet above the roof eave to access the roof. The distance to the ground below was approximately 24 feet.
