29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  On or about March 9, 2018, at 9400 W Commercial Blvd., Sunrise, FL 33351, employees were installing a commercial light fixture, from an aerial lift, during which a body belt/PFAS was not worn and/or a lanyard was not attached to the boom or basket, therefore exposing the employees to approximately 25 feet fall hazard.
