29 CFR 1926.153(o): When damage to LP-Gas systems from vehicular traffic was a possibility, precautions against such damage were not taken:    a) South side of building, on or about 2-26-18:  Motor vehicles were being operated immediately adjacent to two tandemly connected 1,000 gallon Liquefied Petroleum Gas (LPG) tanks that had no protection against accidental contact.     b) Southwest corner of building, on or about 2-26-18:  Motor vehicles were being operated immediately adjacent to a 250 gallon Liquefied Petroleum Gas (LPG) tank that had no protection against accidental contact.     c) West side of building, on or about 2-26-18:  Motor vehicles were being operated immediately adjacent to a 250 gallon Liquefied Petroleum Gas (LPG) tank that had no protection against accidental contact.     d) Southwest corner of building, on or about 2-26-18:  Motor vehicles were being operated immediately adjacent to a 100 gallon Liquefied Petroleum Gas (LPG) tank that had no protection against accidental contact.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access:    a) West side of building, south west corner, on or about 2-26-18:  One employee was using cross braces and scaffold frames for scaffold access, exposing the employee to a fall hazard of up to 17 feet 4 inches.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:    a) West side of building, south west corner, on or about 2-26-18:  One employee was working on two outrigger bracket scaffold platforms without the use of fall protection, exposing the employee to a fall hazard of 18 feet 2 inches and 11 feet 8 inches respectively.     b) West side of building, south west corner, on or about 2-26-18:  One employee was working on an unprotected open sided scaffold platform without the use of fall protection, exposing the employee to a fall hazard of 13 feet 5 inches.     c) West side of building, south west corner, on or about 2-26-18:  One employee was working on a scaffold platform with an unprotected open end of the walk through frame, exposing the employee to a fall hazard of 13 feet 5 inches.
