Employees did not control all hazardous energy sources such as, but not limited to, electrical and pneumatic before performing activities such as, but not limited to, setting-up the machine. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):   A)    Alex Products, Inc., located at 8110 Gasser Road, Paulding, OH 45879: On or about March 6, 2018, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d) and required for the control of the hazardous energy sources such as, but not limited to, electrical and pneumatic were followed before employees conducted activities such as, but not limited to, setting-up the equipment such as, but not limited to, the wire draw machine located at the Draw Line # 2. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work.  As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from amputation hazards while employees conduct activities such as, but not limited to, setting-up the equipment such as, but not limited to, the wire draw machine located at the Draw Line # 2.
