29 CFR 1926.602(a)(9)(ii): Earth moving or compacting equipment which had an obstructed view to the rear was operated in reverse gear; such equipment did not have in operation a reverse signal alarm distinguishable from the surrounding noise level or did an employee signaled that it is safe to do so.  On or about March 14, 2018, and at times prior thereto, the following earth moving equipment did not have an operating reverse alarm and exposed employees to struck-by hazards:   a) CAT D6 Bulldozer   b) CAT 250E #8 Haul Truck   c) CAT 250E #4 Haul Truck   d) John Deere 544 Loader
