29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) Production Area: Lockout procedures were not utilized to protect employees from a hazard created by the in-running rollers on the Midwest Automation glue spreader machine, on or about March 12, 2018.  Or In the alternative:  29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Production Area: Employees were exposed to the unguarded in-running rollers on the Midwest Automation glue spreader machine during servicing operations, on or about March 12, 2018.  ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.147(c)(4)(ii)(A): The Energy Control Procedures did not include a specific statement of the intended use of the procedure:  a) Production Area: The lockout procedure for the Midwest Automation glue spreader machine did not include specific statement that describes when the procedure is to be utilized, on or about March 15, 2018.   ABATEMENT CERTIFICATION REQUIRED
