29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  In the front of the building: Employees engaged in dormer sheeting utilized a ladder that was extended approximately one (1) foot above the eave to access the roof. Employees were exposed to a fall hazard of up to 10 feet to the ground below.

29 CFR 1926.501(b)(13): Employees engaged in residential construction activities 6 feet (1.8m) or more above the lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest systems:  On the roof in the front of the building: Employees engaged in residential dormer sheeting activities from a 6:12 pitch roof were exposed to fall hazards of approximately 10 feet from the eave of the residence to the ground below in that fall protection was not provided/utilized.

29 CFR 1910.178(l)(6): At this establishment, the employer did not certify that each operator has been trained and evaluated to operate the powered industrial truck as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.  The employer did not ensure that a certification of training and evaluation had been created pertaining to the forklift operator.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  In the front of the building at the breaker box/receptacle box: The light green AWG 12/3 extension cord providing electrical power to a DeWalt crosscut saw did not have the outer insulation firmly attached to the male end of the cord.
