29 CFR 1926.1431(k)(2)(ii): The employer did not ensure that each employee was not standing, sitting on or working from the top or intermediate rail or toe board, or use any others means/device to raise their working height.    At the job site, an employee working from a personnel platform attached to a crane was standing on the intermediate rail to raise his working height exposing the employee to fall and electrocution hazards.

29 CFR 1926.1431(n): Hoisting personnel near power lines. Hoisting personnel within 20 feet of a power line that is up to 350 kV, and hoisting personnel within 50 feet of a power line that is over 350 kV, is prohibited, except for work covered by subpart V of this part (Power Transmission and Distribution).  At the job site, the employer failed to ensure that hoisted employees maintained 20 feet of clearance from energized power lines operating at 138kV exposing employees to electrocution hazards.
