29 CFR 1904.39(a)(2): Basic Requirement. The employer failed to report to OSHA within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident:  M/V Golden Alaska:  On or about March 6, 2018 and at times prior thereto, the employer failed to report and amputation within twenty-four (24) to OSHA.  This condition limited the information to the employer, employees, and OSHA the circumstances and corrective actions necessary to ensure a similar incident does not occur in the future.
