29 CFR 1910.28(b)(3)(ii): The employer did not ensure each employee is protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems:    (a) Rock Horeb Marble and Granite, 1031 W 46th Ave, Denver, CO 80211: On and before March 19, 2018, the employer did not ensure that employees were protected from injury, in that a drainage trench in the fabrication area was not guarded by a cover or grate. The trench was approximately 5 inches wide and ran the length of the room. This condition exposed employees to trip hazards.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:  (a) Rock Horeb Marble and Granite, 1031 W 46th Ave, Denver, CO 80211: On and before March 19, 2018, the employer did not ensure that an electrical outlet in the fabrication area had a continuous and effective path to ground. A broken-off grounding prong was stuck inside a 120-volt GCFI electrical outlet, thus preventing a grounded electrical cord from being plugged into the outlet. This condition exposed employees to an electrical shock hazard.
