29 CFR 1910.266(g)(5): Mounting steps and handholds were not provided for each vehicle when necessary to prevent an employee from being injured when entering or leaving the vehicle:  a) On or about March 28, 2018 at logging site, loader: The employer failed to ensure the steps going to the first platform and the cab platform were maintained in a safe condition to prevent employees from slip/trip/fall hazards while getting on or off the loader.  b) On or about March 28, 2018 at logging site, loader: The employer failed to ensure a handhold was maintained to assist employees getting on/off the first platform of the loader to prevent falls of 4 feet 6 inches to the ground.
