29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports:       On or about March 15, 2018, at a new residential construction site located at 6905 N. Grand Blvd in Oklahoma City, OK, an employee was allowed to work on a scaffold that was not fully planked.  The employee was exposed to the hazard of falls.

Grand Blvd in Oklahoma City, OK, an employee was observed working on a scaffold that did not have a ladder or other means of safe access.  The employee was exposed to the hazard of falls.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1m) above a lower level was not protected from falling to that lower level:                                                                                                                                             On or about March 15, 2018, at a new residential construction site located at 6905 N. Grand Blvd in Oklahoma City, OK, an employee was observed working on a scaffold that did not have guardrails around all open ends.  The employee was exposed to the hazard of falls.      NV & T Masonry was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 1230293, citation number 1, item number 4, and was affirmed as a final order on January 7, 2017, with a final abatement date of June 8, 2017, with respect to a workplace hazard located at 1025 W. I-35 Frontage Road Edmond, OK 73034.       NV & T Masonry was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 1188657, citation number 1, item number 2b, and was affirmed as a final order on January 7, 2016, with a final abatement date of November 2, 2016, with respect to a workplace hazard located at 700 S. Broadway Edmond, OK 73034.
