29 CFR 1910.147(c)(2)(ii): When an energy isolating device is capable of being locked out, the employer's energy control program under paragraph (c)(1) of this section did not utilize lockout: a) Doren, Inc. in Wampum, PA. On or about March 20, 2018, employees dismantling the Tiger Machine did not lockout all of the energy sources that were capable of being locked. Components related to the machine were shut off at the electrical panel and the switches were covered with duct tape.

On or about March 20, 2018, six (6) employees were dismantling the Tiger Machine but only one lock was utilized to lockout the equipment.

On or about March 20, 2018, the specific lockout procedure for the Tiger Machine did not identify all energy sources, including but not limited to the potential energy created by the base box and agitator. The procedures did not included methods to restrain the base box during servicing and maintenance.
