29 CFR 1926.1153(e)(2): Where respirator use is required by this section, the employer did not institute a respiratory protection program in accordance with 29 CFR 1910.134:    (a) On or about March 21, 2018 for employees exposed to respiratory hazards while engaged in concrete drilling operations wearing 3M model 6300 negative pressure half-face respirators with p100 particulate filters in absence of medical evaluations at or near 955 South 58th Street in Grand Forks, North Dakota.
