29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:     (a) 407 S Lincolnway Street, North Aurora, Illinois - On or about March 22, 2018, the employer failed to provide training to ensure that an employee working in an elevated boom lift was able to recognize fall hazards and the procedures to minimize fall hazards. The employee was exposed to falling approximately ten (10) feet and did not have his fall arrest lanyard connected to the boom lift, nor was it properly connected to the O-ring on the back of his body harness.       Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
