29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about 3/22/2018, at the above addressed work site, an employee re-roofing a commercial property on a 6:12 pitch roof was exposed to a fall hazard of approximately 25ft while not having a lanyard attached to the harness.
