29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: On or about March 20, 2018, on the roof of a commercial building being constructed where employees were exposed to a fall hazard exceeding fifteen feet when performing steel erection activities without a fall protection system.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. On or about March 20, 2018, at the construction site where employees were exposed to a fall hazard while using an extension ladder to access a roof without extending the ladders side rails at least three feet above the upper landing surface and not providing a grasping device to assist employees in mounting and dismounting the ladder.
