29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) Ground Area (southeast side of structure): On or about March 23, 2018, the employer did not ensure that hard hats were used by employees on the ground while employees on the roof where throwing roofing materials to the ground level exposing employees to struck-by hazards.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems: a) Roof Area: On or about March 23, 2018, and at times prior; the employer allowed his employees to perform roofing activities on a low-slope roof without fall protection exposing his employees to a fall hazard up to approximately fifteen feet (15') to the ground or concrete below.
