29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:      On or about March 23, 2018 the flexible cord connected to the foot pedal that operates the opening and closing of the forge furnace door, was not fitted so that strain relief was provided to prevent the cord casing from pulling away from the cord.

29 CFR 1910.212(a)(3)(iii):     Special hand tools for placing and removing material shall be such as to permit easy handling of material without the operator placing a hand in the danger zone. Such tools shall not be in lieu of other guarding required by this section, but can only be used to supplement protection provided.    On or about March 23, 2018, the employer did not ensure that scale removers used by employees were long enough to enable them to reach the full length of the die without placing their hands or arms between the dies on the Lake Erie 5000 ton press.

Equipment includes, but is not limited to, the Komatsu FG2, serial number FG35516, which was defective in that the clasp used to secure the (LP) cylinder of the powered industrial vehicle, was broken.
