29 CFR 1926.451(a)(6): Scaffolds were not constructed in accordance with the design by a qualified person:    a) Alpine Craftsman and Production L.L.C. at 1136 Steamboat Blvd., Steamboat Springs, CO 80487:  On and before March 22, 2018 the employer did not ensure that a wooden box used to move personnel was designed by a qualified person.  The box was used to move personnel from the ground, by way of a Caterpillar TH360B rough terrain forklift, to various locations on the sides of a two-story single family home while installing windows. This condition resulted in employees being exposed to fall hazards in excess of 10 feet.

29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:  a)  Alpine Craftsman and Production L.L.C. at 1136 Steamboat Blvd., Steamboat Springs, CO 80487:  On and before March 22, 2018 the employer did not ensure that a platform used by a rough terrain forklift to move personnel was properly attached to the forks. A wooden box that was designed to move materials was used to move personnel from the ground, by way of a Caterpillar TH360B rough terrain forklift, to various locations on the sides of a two-story single family home while employees installed windows. This condition resulted in employees being exposed to fall hazards in excess of 10 feet.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided:      (a)   Alpine Craftsman and Productions L.L.C. at 1136 Steamboat Blvd., Steamboat Springs, CO 80487:  On and before March 22, 2017, the employer did not ensure the employees were protected from injury, in that employees engaged in framing activities on the second level of a residential home had exposure to fall hazards from unprotected floor openings around the inside stairwell that lead down to the first level. This condition exposed the employees to a fall hazard of approximately 9-10 feet.      (b)   Alpine Craftsman and Productions L.L.C. at 1136 Steamboat Blvd., Steamboat Springs, CO 80487:  On and before March 22, 2017, the employer did not ensure the employees were protected from injury, in that employees engaged in framing and siding activities on the 2nd level deck on the outside of a residential home had exposure to fall hazards from unprotected sides and edges. This condition exposed the employees to a fall hazard of approximately 9-10 feet.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with one handrail and one stair rail system along each unprotected side or edge:       (a)   Alpine Craftsman and Productions L.L.C. at 1136 Steamboat Blvd., Steamboat Springs, CO 80487:  On and before March 22, 2017, the employer did not ensure that the interior stairway inside the building under construction being used by employees to gain access to the second level, rising approximately 10 feet, was equipped with an adequate stair rail system. Employees utilizing the stairs to reach the second level of the home were exposed to fall hazards of approximately 10 feet.
