29 CFR 1926.451(c)(2)(v):  Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:    Worksite, 207 Front St., Greenport, NY- Employee was lifted and moved horizontally by a Lull Rough Terrain Forklift, 844C-42 while standing on plywood placed on forks.  The employees were using the lift while engaged in framing the exterior approximately 16ft. above the ground below; on or about 3/15/18.    Note:  In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.451(g)(1):  Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    Worksite, 207 front St., Greenport, NY - Employee was standing on plywood placed on forks of a Lull Rough Terrain Forklift, Model 844C-42. The employees were framing the building exterior approximately 16ft. above the ground below. There were no guardrails or personal fall arrest systems in place thus exposing the employee to fall hazards; on or about 3/15/18.    Note:  In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
