29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by ingoing nip points:  (a)  Employees were exposed to crushing/pinch point hazards while working on and around Bakery Line two (2).  The employer did not ensure the conveyor chain and sprocket was properly guarded, in that the barrier guarding had openings that allowed fingers to pass through and contact the chain and sprocket.
