29 CFR 1915.15(a): Preventing hazardous materials from entering. Pipelines that could carry hazardous materials into spaces that have been certified "Safe for Workers" or "Safe for Hot Work" shall be disconnected, blanked off, or otherwise blocked by a positive method to prevent hazardous materials from being discharged into the space:    a) On or about March 7, 2018, Fuel Tank 5-37-1-F was certified "Safe for Workers" when the pipelines connecting Fuel Tank 5-37-1-F to the LCS 15 Fuel System had not been disconnected, blanked off, or otherwise blocked by a positive method when fuel had been added to the LCS 15 Fuel System on March 6, 2018.

29 CFR 1915.163(a)(1): The isolation and shutoff valves connecting the dead system with the live system or systems shall be secured, blanked, and then locked or tagged, in accordance with �� 1915.89, indicating that employees are working on the systems. The lock or tag shall not be removed or the valves unblanked until it is determined that this may be done without creating a hazard to the employees working on the system, or until the work on the system is completed, in accordance with �� 1915.89. When valves are welded instead of bolted, at least two isolation and shutoff valves connecting the dead system with the live system or systems shall be secured, and then locked or tagged, in accordance with �� 1915.89.  a)  On or about March 7, 2018, the employer's usage of valve FT-V-40 to isolate Fuel Tank 5-37-1-F from the LCS 15 Fuel System during the Fuel Transfer Pump test did not meet the requirement of, at least, two isolation and shutoff valves being secured to isolate a section of a piping system.

29 CFR 1915.15(b): Alteration of existing conditions. When a change that could alter conditions within a tested confined or enclosed space or other dangerous atmosphere occurs, work in the affected space or area shall be stopped. Work may not be resumed until the affected space or area is visually inspected and retested and found to comply with 1915.12, 1915.13, and 1915.14 of this part, as applicable:   (Note to paragraph (b): Examples of changes that would warrant the stoppage of work include: The opening of manholes or other closures or the adjusting of a valve regulating the flow of hazardous materials.)    a) On or about March 6, 2018, Fuel Tank 5-37-1-F was not re-evaluated to determine Fuel Tank 5-37-1-F had been disconnected, blanked off or otherwise blocked by a positive method from the LCS 15 Fuel System after fuel was added to the vessel when a Coating Department employee had been assigned to perform Final Cleaning work tasks in Fuel Tank 5-37-1-F on or about March 7, 2018.

29 CFR 1915.89(g)(1): Before servicing machinery, equipment, or a system that has a lockout/tags-plus system, the employer shall ensure that the authorized employee, or the primary authorized employee in a group lockout/tags-plus application, verifies that the machinery, equipment, or system is deenergized and all energy sources isolated:    a) On or about March 7, 2018, a Coating Department employee was not notified of a new energy control verification requirement while entering Fuel Tank 5-37-1-F on LCS 15 to perform Final Cleaning work tasks. The Coating Department employee had received a Tank Re-entry Form indicating no lockout procedure was required to enter the Fuel Tank 5-37-1-F on or about March 1, 2018. On or about March 6, 2018, fuel was added to the LCS 15 Fuel System and Fuel Tank 5-37-1-F had not been disconnected, blanked off, or otherwise blocked by a positive method to prevent fuel entering the Fuel Tank 5-37-1-F during and/or after the ship fueling.
