29 CFR 1910.119(j)(4)(iii): The frequency of inspections and tests of process equipment was not consistent with applicable manufacturers' recommendations and good engineering practices, and more frequently if determined to be necessary by prior operating experience:    a)  Pad 5, 150 Firmenich Way, Newark, NJ    The employer manufactured Habanolide by mixing highly hazardous chemicals including N-Butyl Acetate, Bicyclene Oxide and 70% Hydrogen Peroxide, in reaction vessels V-570 that contains 13,886 lbs of flammable mixture, V-57AO that contains 13,886 lbs, and V-580 that contains 20,530 lbs and an on site 70% Hydrogen Peroxide tank that contains a maximum of 20,020 lbs, on or about March 28, 2018 to September 18, 2018.     1 - The employer failed to conduct external visual inspections on vessel V-570, V-57A0 and V-580 within a 5 year interval as required by industry standards, such as but not limited to API 510-2014, Section 6.4.1.     Violation determined on or about June 13, 2018    2 - LSHH-571108 Switch Level, High-High, T-5711 requires preventive maintenance once every year.  The employer performed the preventive maintenance on this device on 4-21-17, then again on 8-13-18, exceeding the annually required preventative maintenance interval.    Violation determined on or about August 8, 2018

The vessel was rerated June 2, 1994 from 250 PSI to 150 PSI because of a new valve welded into the opening. However, the employer failed to update the vessel's nameplate with required information as required by industry standards, such as but not limited to API 510-2014, Section 8.2.2.    Violation determined on or about June 13, 2018
