29 CFR 1918.83(b): When breaking down stowed cargo, precautions were not taken to prevent remaining cargo from falling.    On or about March 28, 2018, at the Port of Houston, City Dock 20 where employees were exposed to struck-by and crush-by hazards while unloading bundles of pipe without the employer taking precautions to prevent the remaining bundles of pipe from falling.
