The point of operation was not guarded during the entire production/operating cycle.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: a) First State Container, LLC - On or about April 6, 2018, the employer failed to ensure that each authorized employee affixed a personal lockout device to the group lockout devices prior to performing service or maintenance. A personal lock was improperly placed on the group lockout device, rendering the lockout device ineffective when servicing the Rocheleau Blow Molding System Machine.
