29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    a.  On or about April 3rd, 2018, at the above addressed job site, employees were exposed to a struck-by hazard while working in the vicinity of a forklift operator that had not been trained and certified.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:     a.  On or about April 3rd, 2018, at the above addressed job site, employees were not protected from an approximate   24-feet fall hazard. Employees were located on the third floor of a residential structure under construction and in the process of dumping trash into a dumpster.

29 CFR 1926.600(a)(3)(i): Bulldozer and scraper blades, end-loader buckets, dump bodies, or similar equipment were not fully lowered or blocked when being repaired or when not in use:    a.  On or about April 3rd, 2018, at the above addressed job site, an employee was exposed to a crushing hazard while working near a fork truck that had been left unattended with the forks raised approximately 24 feet and a dumpster that was attached to it.
