29 CFR 1904.41(a)(1): An establishment which had 250 or more employees at any time during the previous calendar year, and this part required the establishment to keep records, the employer did not electronically submit information from the three recordkeeping forms that you keep under this part (OSHA Form 300A Summary of Work-Related Injuries and Illnesses, OSHA Form 300 Log of Work-Related Injuries and Illnesses, and OSHA Form 301 Injury and Illness Incident Report) to OSHA or OSHA's designee. The employer did not submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the forms:    (a)  Energes Services LLC, at 1328 E 18th Street, Greeley, CO: On and preceding 4/3/18, the employer did not submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the forms.  The employer documented for the establishment at 1328 E 18th Street, Greeley, CO, 276 employees on its 2016 OSHA 300A Summary Sheet and 375 employees on its 2017 OSHA 300A Summary Sheet.  The employer documented four recordable injuries on the establishment's 2016 OSHA 300 Log.  The employer did not electronically submit the establishment's 2016 OSHA 300A Summary Sheet by December 31, 2017.
